GENDER AFFIRMING CARE

Care for adults

The Project of the Quad Cities is committed to continuing to offer and provide gender-affirming care for individuals 18 and over. At this time, this may include:

  • Gender-affirming behavioral health services

  • Referral to external medical provider who prescribes gender-affirming hormone therapy (GAHT)

  • Letter of Support for GAHT and/or surgery

Scheduling and Preparing for Your Appointment ​ 

  1. Call the Behavioral Health and Gender Affirming Care Office Coordinator, Salena, at 309-581-2027 to schedule your appointment.  

  2. If seeking GAHT, we may schedule an Initial Gender Affirming Care (GAC) Consultation, with our Transgender and Gender Expansive Care Coordinator, via telehealth or in-person, if additional information is needed to determine the appointment type.

  3. Review and complete the Adult Packet including a release of information.

The Project knows that gender affirming care is lifesaving care. That’s why we’re proud to serve both adults and youth at our Illinois-based clinic.

What is gender affirming care?

Gender-affirming care (GAC) is a range of healthcare services (medical, social, psychological, and behavioral) interventions that support people to live authentically as their gender identity. Every person is unique, so GAC can look different for from one person to another and change over time.

Scheduling and Preparing for Your Appointment ​ 

  1. Call the Behavioral Health and Gender Affirming Care Office Coordinator, Salena, at 309-581-2027 to schedule your appointments  

  2. Review and complete the GAC packet including a release of information

  3. Complete an initial Medical Appointment 

  4. Behavioral Health, if needed 

  5. Optional

    • Consult with Transgender and Gender Expansive Care Coordinator 

    • Get a referral from your current Primary Care Provider (if applicable). Be sure to have a copy of your health records sent to The Project of the Quad Cities. Referrals and records can be faxed to 309-762-4481
